  • The name you used at Silver Creek

  • Your date of birth

  • The name and address where the diploma will be sent

  • Your phone number

  • A copy of your photo I. D.

  • $50 fee

***Orders typically will take 10 business days once graduation status has been verified by the Silver Creek High School's Registrar.
